As a Telehealth Nurse Practitioner with YSP, you will deliver comprehensive virtual care to patients across the U.S. in your area of specialty. You will work independently, supported by a robust digital infrastructure and a collaborative network of healthcare professionals. YSP provides all digital resources, including EMR/EHR, EDI, malpractice insurance, billing/coding support, and access to collaborative physicians.
Family Medicine / Primary Care NPs: - Pre-care: Acute triage, health screening, pre-visit assessments, medication reconciliation.
- Post-care: Hospital discharge follow-up, medication management, post-procedure check-ins.
- Routine care: Chronic disease management (diabetes, hypertension), annual wellness visits, preventive screenings, behavioral health check-ins.
Psychiatric-Mental Health NPs:
- Pre-care: Initial mental health assessment, crisis triage, intake evaluations.
- Post-care: Therapy follow-up, medication adjustment, relapse prevention, post-hospitalization check-ins.
- Routine care: Ongoing psychotherapy, medication management, support groups, behavioral health monitoring.
Pediatric NPs:
- Pre-care: Developmental screening, acute illness triage, parental guidance.
- Post-care: Follow-up after hospital discharge, medication management, and post-procedure monitoring.
- Routine care: Well-child visits, chronic pediatric condition management (asthma, ADHD), behavioral health, specialty consultations.
Geriatric NPs:
- Pre-care: Cognitive and functional assessments, medication review, pre-visit planning.
- Post-care: Post-acute care monitoring, medication titration, caregiver support.
- Routine care: Chronic disease management, fall risk assessment, preventive care, and advance care planning.
Women’s Health / OB-GYN NPs:
- Pre-care: Prenatal consultations, pre-op counseling, reproductive health screening.
- Post-care: Postpartum check-ins, post-op follow-up, medication management.
- Routine care: Birth control management, STI counseling, routine prenatal visits, menopause management.
Internal Medicine NPs:
- Pre-care: Triage for acuity, pre-procedure assessment, risk stratification.
- Post-care: Chronic disease follow-up, medication titration, post-hospitalization care.
- Routine care: Hypertension, diabetes, hyperlipidemia management, preventive screenings.
Cardiology NPs:
- Pre-care: Initial evaluation for cardiac symptoms, pre-op cardiac clearance.
- Post-care: Post-stent or post-procedure follow-up, medication adjustment, symptom monitoring.
- Routine care: Congestive heart failure management, arrhythmia monitoring, risk factor modification.
Neurology NPs:
- Pre-care: Headache/migraine triage, initial assessment for movement disorders.
- Post-care: Post-stroke follow-up, medication management, rehabilitation support.
- Routine care: Epilepsy, Parkinson’s disease, cognitive disorder management.
Oncology NPs:
- Pre-care: New patient care planning, pre-op planning, symptom assessment.
- Post-care: Chemotherapy follow-up, symptom management, survivorship care.
- Routine care: Ongoing cancer management, palliative care, patient education.
Other Specialties (e.g., Endocrinology, Dermatology, Pulmonology, Infectious Disease):
- Pre-care: Initial consults, risk assessment, pre-procedure counseling.
- Post-care: Follow-up after interventions, medication management, and lab review.
- Routine care: Chronic disease management (e.g., diabetes, thyroid, asthma, HIV), preventive care, patient education.
- Triage and initial assessment.
- Diagnosis and treatment planning.
- Medication management and refills.
- Chronic disease management.
- Behavioral health therapy and counseling.
- Patient education and self-care coaching.
- Remote patient monitoring (reviewing home device data).
- Care coordination and referrals.
- Pre-procedure and pre-op consultations.

- Support for caregivers and families.
Interprofessional Collaboration:
- Collaborative Practice: Work closely with YSP’s network of collaborative physicians, billers/coders, and other healthcare professionals using secure digital platforms.
- Digital Resources Provided: EMR/EHR, EDI, telehealth platform, patient scheduling, billing/coding, malpractice insurance, and technical support.
- Compliance: All practice is in accordance with federal and state laws, including licensure, scope of practice, and telehealth regulations.
Requirements:
- Active, unencumbered NP license in the state(s) where you will provide care.
- Board certification in your specialty.
- Experience in telehealth preferred (training provided).
- Strong communication and digital literacy skills.
- Ability to work independently and manage your own patient panel.
- Reliable internet and secure, private workspace.
- Malpractice insurance (provided by YSP).
- Ability to comply with all documentation, billing, and regulatory requirements.
