Comfort Home Care is in search of per-diem RNs and LPNs in Acton, Concord, and the surrounding areas. Our nurses visit a diverse population, mostly with mental health diagnoses, performing nursing assessments, communicating with other providers, and maintaining patients in the community as independently as possible.

Day to day responsibilities of a Comfort Home Care nurse:

Provide initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
Initiate the plan of care, necessary revisions, and perform OASIS assessments at appropriate time points when applicable.
Initiate and perform appropriate nursing interventions.
Coordinate services and inform physicians and other personnel of changes in the client's condition and needs.
Collaborate with patient /family and other health care providers and/or community resources with planning of care and discharge.
Prepare clinical and progress notes and submit all required paperwork in a timely manner according to agency policy and procedure.
