1:1 School Based - Lpn Licensed Practical Nurse Carbondale, PA area Job Details for the Lpn Licensed Practical Nurse:

• Local School District; Life Skills Classroom
• Up to $29 per hour
• Monday through Friday, hours between 7 am and 4 pm

Essential Functions for the Lpn Licensed Practical Nurse:

• Participates in Ongoing Interdisciplinary Assessment of the Patient.
• Participates in the Development and Evaluation of the Plan of Care in Partnership with the Patient, Representative (If Any), and Caregiver(S).
• Recognizes and Reports Changes in the Patient’s Condition to the Rn Supervisor.
• Counsels and Educates the Patient and Family in Meeting Healthcare Related Needs.
• Completes Timely and Accurate Clinical Notes Including Addressing Patient’s Progress.
• Assists the Rn in Carrying out the Patient Plan of Care.
• Assists the Client in Learning Appropriate Self-care Techniques.
• Follows Accepted Standards of Nursing Practice.
• Participates in In-services or Training, As Requested.
• Participates in the Office’s Emergency Preparedness Program

Minimum Requirements for the Lpn Licensed Practical Nurse:

• Current License As a Lpn/Lvn in the State(S) in Which He/She Practices
• Minimum One (1) Year Relative Experience; Required
• Cpr Certification
• Valid Driver's License
• Willing to Undergo Pre-employment Screenings
