Location: New York, NY 10032 (Onsite with some remote flexibility for administrative work)
Schedule: Monday–Friday, days; no on-calls or weekends. Clinic hours start around 8:30-9:00 AM; arriving by 8 AM preferred. Typical workday until at least 5 PM.
Salary: $135,000–$160,000

Position Overview
Seeking a skilled and adaptable Medical NP or PA to join a leading Kidney Transplant Program focused on the medical care and post-operative management of transplant recipients. This role supports nephrologists in managing patients from immediately post-transplant through long-term follow-up (up to 5+ years).
The position is a “dual role,” combining in-clinic patient care with care coordination, medication management, and telehealth follow-ups. The program is fast-paced, evolving, and offers opportunities for professional growth.

Key Responsibilities
For NP:

• Manage outpatient care for kidney and pancreas transplant recipients, post-op donors, and patients after surgical procedures.
• Perform comprehensive history and physical exams, develop care plans, and document patient progress.
• Conduct pre-operative assessments and obtain consents.
• Order, interpret labs and diagnostics, and manage medication authorizations.
• Develop clinical protocols, patient education materials, and participate in research monitoring.
• Collaborate with multidisciplinary teams to improve patient outcomes and program quality.

For PA:

• Coordinate medical and surgical management of complex liver, intestinal, and transplant patients in outpatient settings.
• Conduct new patient evaluations and consultations.
• Provide education on liver disease and transplantation to patients and staff.
• Manage immunosuppressant therapy and medication adjustments based on labs.
• Provide telephone medical advice and collaborate with primary physicians.
• Assist with prior authorizations for medications and testing.
• Participate in outpatient on-call rotation and care coordination meetings.

Qualifications
Nurse Practitioner (NP):

• Master of Science in Nursing with current NYS RN and NP licenses.
• ANCC Board Certification required.
• BLS and DEA certifications required.
• Strong interpersonal and communication skills.
• Proficiency with MS Office and technology platforms.

Physician Assistant (PA):

• Graduate of ARC-PA accredited or NY State-approved PA program.
• Current NYS PA License and NCCPA Board Certification required.
• BLS and DEA certifications required.
• Strong interpersonal and communication skills.
• Proficiency with MS Office, EMR systems, and technology platforms.

Job Type: Full-time

Pay: $135,000.00 - $160,000.00 per year
