About the Role
We are seeking a compassionate and detail-oriented Care Management Nurse (RN or LPN) to join our team. In this role, you will collaborate with healthcare professionals to develop and oversee personalized care plans, ensuring patients receive comprehensive and coordinated care. You will serve as a key point of contact for patients and their families, helping them navigate their health journey with confidence and support.

Key Responsibilities
Develop, implement, and monitor individualized care plans tailored to patients' health and wellness goals.
Act as a primary resource for patients and families, providing regular updates and addressing concerns with empathy and professionalism.
Work closely with healthcare providers, social workers, and other professionals to ensure a holistic approach to patient care.
Maintain accurate and up-to-date electronic health records, documenting care plans, patient interactions, and any changes in health status.
Manage high-risk and chronically ill patients through Chronic Care Management, using health coaching techniques to promote self-management and lifestyle improvements.
Demonstrate strong organizational and interpersonal skills to effectively collaborate with healthcare teams.
Qualifications & Requirements
Graduate of an accredited nursing program.
Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Montana (or temporary license).
Experience caring for patients with chronic diseases.
Certifications & Licenses:
Valid Montana Driver's License.
Current CPR certification.
