Join our team as a Primary Care Nurse Practitioner and deliver high-quality, patient-centered care in a collaborative rural outpatient setting, providing comprehensive assessments, wellness visits, chronic disease management (diabetes, hypertension, COPD), acute care for minor illnesses and injuries, preventive screenings, vaccinations, medication management, and evidence-based treatment plans. Daily responsibilities include conducting thorough histories and physicals, ordering and interpreting labs and diagnostic imaging, e-prescribing, coordinating care with physicians and specialists, triaging patient concerns, managing refill requests, documenting efficiently in the EHR, delivering telehealth visits, educating patients on lifestyle and self-management, and contributing to quality improvement and population health initiatives.
