Job Title: PER VISIT REGISTERED NURSE/HOME CARE

POSITION SUMMARY:

The Home Health Nurse assesses, plans, coordinates, implements and evaluates plans of care for patients in their homes, in accordance with professional standards, regulatory guidelines, and agency policies. This includes ongoing communication with health team members to provide coordination and continuity of care.

MINIMUM REQUIREMENTS

• License and current registration to practice as a Registered Nurse in New York State.
• An associate’s Degree in Nursing from an approved program is required: Baccalaureate Degree in Nursing is preferred.
• Two (2) years of recent medical-surgical experience in a hospital setting.
• Must have a valid   driver's license and the ability to provide own transportation during working hours for the purpose of making home visits.
