Your Role:

• Manage a primary caseload of home health patients and deliver skilled nursing care.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Implement and regularly reevaluate care plans to ensure optimal patient outcomes .
• Work alongside an incredible interdisciplinary team to provide holistic care.

What You Need to Succeed:

• RN license in Pennsylvania (Required).
• Graduate of an accredited nursing program (BSN preferred).
• 1+ years of nursing experience (Home health, med-surg, or critical care preferred).
• Strong clinical skills and a passion for patient-centered care.
• Reliable transportation & valid driver’s license (because home visits matter!).
• CPR certification.
