Job Description

We are seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our growing team. In this role, you will provide high-quality nursing care to patients in the comfort of their homes. You will be responsible for assessing patients’ health, developing individualized care plans, administering treatments, and ensuring continuity of care in collaboration with a multidisciplinary team. Responsibilities:

• Assess and monitor patients’ responses to intravenous (IV) medications and treatments in a home care setting.
• Provide clear, compassionate explanations of procedures while continuously monitoring and documenting vital signs.
• Administer prescribed IV therapies, hydration, medications, and blood products with accuracy and efficiency.
• Collaborate closely with physicians and manage infusion equipment to ensure safe and effective patient care.
• Respond promptly to emergency situations, initiating appropriate interventions and contacting emergency services when necessary.
• Promote a professional, safe, and comfortable environment to enhance the patient experience in the home setting.
• Apply strong knowledge of pharmacology and adhere to best practices in documentation and patient records.
• Follow rigorous infection control protocols to prevent the spread of disease and maintain patient safety.
• Leverage advanced clinical skills acquired through nursing education and experience to deliver exceptional patient care.

Qualifications:

• Highly proficient in initiating and managing peripheral IVs, arterial lines, and various intravenous access devices.
• Skilled in operating and monitoring infusion equipment while ensuring line patency and functionality.
• Experienced in assessing patient responses to medications and promptly identifying potential drug reactions or complications.
• Strong adherence to accurate documentation practices and strict infection control protocols.
• Demonstrates excellent clinical skills developed through comprehensive nursing education and hands-on experience.
• Competent in setting up and monitoring IV infusions, administering injections, performing tube feedings, and managing wound care, including dressing changes.
