All while understanding that **together** we are even better. **The role you'll contribute:** Ensures timely communication with patients, their families, and healthcare providers through professional collaboration. Collaborates with the Community Care team to evaluate patients' medical and psychosocial conditions and assess their needs. Develops and documents patient-centered plans of care based on assessment, diagnosis, and patient profiles. Evaluates and modifies plans of care based on patient responses and attainment of expected outcomes. Adheres to established standards of practice and the National Association of Social Workers' code of ethics. Provides leadership and support, organizing program materials and promoting care coordination. Assists with community outreach educational programs related to care coordination. Coordinates care across the continuum to ensure safe, efficient, and cost-effective resource utilization. Identifies patient populations needing care coordination and addresses their health-related needs, including social determinants of health. Maintains an active caseload of "at risk" patients and optimizes their independence through education and community resources. Provides crisis intervention and community support resources for patients and families experiencing significant stress due to illness. Other duties as assigned. **The value you'll bring to the team:** Responsible for working collaboratively with community care nurses, physicians, staff, and other healthcare professionals to provide care coordination across the healthcare continuum for patients within the program. This position ensures that members with complex issues or high utilization are able to navigate the healthcare continuum while improving patient and family experience. This role eliminates care gaps by addressing the social determinants of health. In addition, this position coordinates a wide range of community-based and healthcare support services for members. _*_ **_Terms:_** _12 month Employment Contract Required for Relocation and sign on bonus_ **ReRelocation Eligibility :** Candidate must be relocating greater than 50 miles for the purpose of employment at a CFD facility. **Qualifications - External** **The expertise and experiences you'll need to succeed:** **Minimum qualifications** : Masters in Social Work (MSW) Minimum two (2) years experience in hospital/medical social work **Preferred qualifications:** Care Management discharge planning experience Knowledge of state and federal guidelines pertinent to care management Licensed Clinical Social Worker (LCSW) ACM/CCM certification This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
