Help at Home is Hiring a Clinical Assistant! **This role will be fully remote in Indiana with a competitive hourly rate of $21/hour.** The Clinical Assistant will organize patient care and treatments by including all members of the patient's care team. They will assist RN Case Managers in duties such as care coordination, order tracking, filing, and HHA education. We are looking for someone detail oriented and someone looking to be a part of a team. **Qualifications** + CMA/QMA preferred + Two years' experience in care coordination + Preferred two years' experience with chart + Auditing preferred Knowledge of service + Regulations and process Ability to exercise + Independent judgment. + Phone/computer experience. + Ability to demonstrate above accuracy attention to detail to ensure accuracy. Ability to read and interpret medical record data for chart audit. **Responsibilities** + All tasks as directed by RN Case Manager, or Area Clinical Leader. + Care coordination with physicians and other healthcare entities. This includes, but is not limited to: + Face to Face + H&P + Transfer summaries + Business Associate Agreement + Sharing of POC with other entities o Expirations + QAPI + Tracking of frequency and duration of services provided with proper documentation of missed visits. + Quality Assurance of HHA daily visit sheets and education as applicable. + Tracking of supplemental physician orders and plans of care. + Maintain physician expiration; license, NPI, and Medicaid. + Input of basic non-clinical information into the EMR. **Working environment** Fully Remote - Indiana Data Security and Privacy Statement At Help at Home, we prioritize protecting your personal information during the hiring process.
